It's clear from the reactions of the onlookers they are seeing something that looks like a miracle; none of them are looking down, they are instead looking in amazement at something that is floating by them. This is not how they would react if the midget has simply fallen from the trapeze and hit the ground. This means something highly unusual happens.

My take is that what dies is his longing for the girl - all he wants is her and in the end he realizes he could never have her. This strikes him most strongly when he is just about to give her the ring and her lover appears from the wagon, his shirt off, something that magnifies his physical normalcy. Perhaps thinking this athletic prowess is what attracts the girl to the other man, the midget attempts to use the trapeze to prove that he also is worthy of her love.
