DCR or dacryocystorhinostomy operations are done to relieve a watery, sticky eye caused by a blockage between the tear sac at the corner of the eye and the tear duct into the nose. The aim of surgery is to make a new passage from the tear sac into the nose. This bypasses the blockage and allows the tears to drain normally again. DCR surgery is worth undertaking if the watering is bad enough to interfere with patient's daily life. It is also recommended if patient has had an attack of acute dacryocystitis (infection in the tear sac) as a result of a blocked tear duct; otherwise patient may have repeated attacks of a red, painful swelling at the corner of the eye. The surgery can be performed in two ways: · Externally -- through the skin · Endoscopically -- from within the nostril. External DCR: A small cut (1-2 cm) is made on the side of the nose, approximately where a pair of glasses rests. A piece of bone between the tear sac and the nose is removed to make a new channel for the tears. Some surgeons use (Not shown in this video) a soft silicone tube or cord to keep the tear passage open during healing. This tube is not usually noticeable and is removed in clinicabout 12 weeks after the operation. The operation takes about 1-1.5 hours, so is NOT a minor procedure. The success rate for this operation is 85-90%. Success means that the watering stopscompletely or only happens in very windy weather. In thisvideo, Dr Suresh K Pandey and Dr Vidushi Sharma, Eye surgeons for Suvi Eye Institute, Kota, India demonstrate various phaco techniques for managing soft cataract. Plenty of videos are available demonstrating phaco techniques for hard cataracts; but, in many ways the soft cataracts are the most challenging. Soft cataracts tend to jump to the phaco needle which can lead to capsule rupture. Soft cataracts are hard to crack which can make the divide and conquer very difficult. With soft cataracts, the inexperienced surgeon may unexpectedly sculpt through the nucleus too rapidly, as well as aspirate and possibly phaco the posterior capsule. In this video we demonstre few strategies for soft cataracts to increase safety for these tricky lenses. The simplest strategy for very soft cataracts is to simply prolapse the soft lens into the iris plane/anterior chamber. Even better is to prolapse just the nucleus. First we like to hydrodissect, careful to keep the cannula over the lens, to avoid prolapse. then spin the lens. Then perform hydrodelineation and allow the nucleus to prolapse into the iris plane/anterior chamber. In a perfect world you can just prolapse the nucleus (sometimes soft lenses have a denser central core) and leave the epinuclear material in the bag. Then remove the nucleus with the phaco needle with epinuclear settings (linear control of vacuum 0-350, asp at like 30-35 fixed, little power). A darkly brunescent nucleus, with the color of molasses or a cola drink, presents special surgical challenges. We must select a surgical strategy to manage these cases. When the red reflex is insufficient to allow adequate visualization of the anterior capsular edge, staining the anterior capsule is a critical step in performing successful cataract surgery. A stained peripheral anterior capsule also facilitates later phacoemulsification. The surgeon who can see the anterior capsular edge is less likely to nick it with the ultrasound tip or misplace a phaco chopping instrument on top of the anterior capsule. Protecting the corneal endothelium is especially important during the phacoemulsification of dense nuclei, because the surgery is prolonged and requires more manipulation and ultrasound power.
