Colgan Air Flight 3407, marketed as Continental Connection under a codeshare agreement with Continental Airlines, was a daily US regional airline commuter flight from Newark Liberty International Airport in New Jersey to Buffalo Niagara International Airport in New York State. A Bombardier DHC8-402 Q400 operating as Flight 3407 departed late from Newark on February 12, 2009, at 9:20 pm EST. Pacific Southwest Airlines (PSA) Flight 182, registration N533PS, was a Boeing 727-214 commercial airliner that collided with a private Cessna 172 over San Diego, California on September 25, 1978. Pacific Southwest Airlines' first accident involving fatalities, the death toll of 144 makes it the deadliest aircraft disaster in California history. It was also the deadliest plane crash in the history of the United States until American Airlines Flight 191 went down eight months later. The Boeing and Cessna crashed into North Park, a San Diego neighborhood, killing all 135 on board, the two men aboard the Cessna, and seven people on the ground, including two children. Nine others on the ground were injured and 22 homes were destroyed or damaged. The midair collision contributed to San Diego's Lindbergh Field airport being ranked 10th among the world's Most Extreme Airports in a two-hour documentary of the same name released in July 2010, which aired in the US on the History Channel. The PSA 182 accident caused the revision of air traffic rules applicable to the busiest airports across the US, with the intention of improving separation of aircraft operating in the vicinity of large airports.

The instrument flight rules scheduled international flight was in cruise flight at FL350 with the autopilot engaged, when it abruptly rolled into a 30 to 40 degree left bank. There were indications that the lower rudder moved to the left blowdown limit, and remained there. Emergency procedures failed to correct the problem, and the flight diverted to the nearest airport for an emergency landing. The data retrieved from the FDR showed an initial lower rudder deflection of 17.5 degrees left, and a subsequent increase to 32 degrees (full) left. The incident airplane has two independently supported and operated rudders, which provide yaw control. Typically, the upper and lower rudders operate in unison.
