On January 27, 1967, tragedy struck the Apollo program when a flash fire occurred in command module 012 during a launch pad test of the Apollo/Saturn space vehicle being prepared for the first piloted flight, the AS-204 mission. Three astronauts, Lt. Col. Virgil I. Grissom, a veteran of Mercury and Gemini missions; Lt. Col. Edward H. White, the astronaut who had performed the first United States extravehicular activity during the Gemini program; and Roger B. Chaffee, an astronaut preparing for his first space flight, died in this tragic accident. The hatch was held closed by several clamps which required extensive ratcheting to release. Under the best of circumstances, it could take at least 90 seconds to open. Since it opened inward, pressure had to be vented before the hatch could be forced open. It was nearly 5 minutes after the start of the fire before rescuers could get into the cabin. By this time, the oxygen rich atmosphere which had seeped into the materials of the cabin had caused the fire to spread rapidly. Although the ignition source of the fire was never conclusively identified, the astronauts' deaths were attributed to a wide range of lethal design hazards in the early Apollo command module. Among these were the use of a high-pressure 100 percent-oxygen atmosphere for the test, wiring and plumbing flaws, inflammable materials in the cockpit an inward-opening hatch that would not open in this kind of an emergency and the flight suits worn by the astronauts.

Apollo 1 is the official name given to the never-flown Apollo/Saturn 204 (AS-204) mission. Its command module (CM-012) was destroyed by fire during a test and training exercise on 27 January 1967 at Pad 34 (Launch Complex 34, Cape Canaveral, then known as Cape Kennedy) atop a Saturn IB rocket. The crew onboard were the astronauts selected for the first manned Apollo program mission: Command Pilot Virgil I. "Gus" Grissom, Senior Pilot Ed White and Pilot Roger B. Chaffee. All three died in the fire. Although the ignition source of the fire was never determined, the astronauts' deaths were attributed to a wide range of lethal design hazards in the early Apollo command module. Among these were its use of a highly-pressurized 100%-oxygen atmosphere, many wiring and plumbing flaws, flammable materials in the cockpit, a hatch which might not open at all in an emergency, and the flight suits worn by the astronauts.

Causes of the fire included the use of a 100% oxygen atmosphere, much flammable material used in the cockpit and many sources of ignition present. After the fire the Apollo project was grounded. In hindsight the command module was understood to be extremely hazardous and in some instances, carelessly assembled. Many design changes were made. In March 1961, Soviet cosmonaut Valentin Bondarenko was killed when a fire started in the pure-oxygen atmosphere of an isolation chamber but the USSR concealed this tragedy for over 20 years. However, the design hazards of a 100% oxygen environment had been well described by 1967 and many deaths from flash fires in pure oxygen at or near sea-level pressure had been publicly reported during the 1950s and 60s. A 1966 editorial in the journal Space/Aeronautics asserted "The odds are that the first spaceflight casualty due to environmental exposure will occur not in space, but on the ground", and further noted that safety protocols for the Apollo project were thoroughly lacking. On January 27, 1967, tragedy struck the Apollo program when a flash fire occurred in command module 012 during a launch pad test of the Apollo/Saturn space vehicle being prepared for the first piloted flight, the AS-204 mission. Three astronauts, Lt. Col. Virgil I. Grissom, a veteran of Mercury and Gemini missions; Lt. Col. Edward H. White, the astronaut who had performed the first United States extravehicular activity during the Gemini program; and Roger B. Chaffee, an astronaut preparing for his first space flight, died in this tragic accident. A seven-member board, under the direction of the NASA Langley Research Center Director, Dr. Floyd L. Thompson, conducted a comprehensive investigation to pinpoint the cause of the fire. The final report, completed in April 1967 was subsequently submitted to the NASA Administrator. The report presented the results of the investigation and made specific recommendations that led to major design and engineering modifications, and revisions to test planning, test discipline, manufacturing processes and procedures, and quality control. With these changes, the overall safety of the command and service module and the lunar module was increased substantially. The AS-204 mission was redesignated Apollo I in honor of the crew.

During a preflight test for what was to be the first manned Apollo mission, a fire claimed the lives of three US astronauts; Gus Grissom, Ed White and Roger Chaffee. After the disaster, the mission was officially designated Apollo 1. At 1 pm on Friday, 27 January 1967 the astronauts entered the capsule on Pad 34 to begin the test. A number of minor problems cropped up which delayed the test considerably and finally a failure in communications forced a hold in the count at 5:40 pm At 6:31 one of the astronauts (probably Chaffee) reported, "Fire, I smell fire." Two seconds later White was heard to say, "Fire in the cockpit." The fire spread throughout the cabin in a matter of seconds. The last crew communication ended 17 seconds after the start of the fire, followed by loss of all telemetry.
