a film about traveling with double vision. I had the misfortune of one of my eyes not fully moving to one side. The current diagnosis for this is 'internuclear ophthalmoplegia', which apparently is a disorder of conjugate lateral gaze in which the affected eye shows impairment of adduction. My left eye diverges from the right eye, which means that my brain "sees" double, or what they call 'diplopia'. As my eyes no longer work together they see different images and my brain cannot comprehend the resulting information. This has led me to see things that aren't there, resulting in my tripping over and walking into objects. In this new work I have continued my interest in creating moving pictures. Painters, photographers, and filmmakers have inspired its making and I have treated these animated landscapes as a simplified drawing. Trying to avoid putting in more than is necessary; I endeavour to balance the identity of the landscape, with the abstraction of the picture frame, reducing and simplifying the colour space. The soundtrack is constructed from sounds I recorded inside the car; driving along different tarmac textures. I have also used a recording in a lay-by on the A30. These sounds have been mixed and heavily treated.

Ectopic pupil repair by Steven G. Safran MD

This patient was a 52 year old who had a ruptured globe 40 years earlier with the top third of his iris incarcerated in the wound. He then had cataract surgery done elsewhere 3 years previously and had monocular diplopia and poor vision because he was looking through the haptic optic junction. There were vitreous strands adherent to the iris where it was incarcerated seen on gonioscopy that were treated with a YAG laser prior to this surgery. My goal was to reposition his pupil and preserve the 2/3 of a functioning iris sphincter that he had inferiorly so that he would still have pupil function. We did achieve that goal along with a marked improvement in cosmesis, resolution of monocular diplopia and 20/20 corrected Va. A YAG capsulotomy was done one month after this procedure was performed. Note the use of iris retractors to pull the inferior pupil down as the superior pupil is sutured closed. You can see the inferior 2/3 of iris sphincter has been preserved throughout the procedure. Also note that I'm sitting on the temporal side during this surgery and do adjust my seating position a bit during the case rotating the microscope to allow a better "angle of attack". Please note that if you turn the screen 90 degrees counterclockwise at the end of the procedure to view it in the normal "slit lamp" orientation you can see that the centration of the pupil is excellent when the case is finished.
Internuclear Ophthalmoplegia (INO)

Internuclear ophthalmoplegia (INO) is a disorder of conjugate lateral gaze in which the affected eye shows impairment of adduction. When an attempt is made to gaze contralaterally (relative to the affected eye), the affected eye adducts minimally, if at all. The contralateral eye abducts, however with nystagmus. Additionally, the divergence of the eyes leads to horizontal diplopia. That is, if the right eye is affected the patient will "see double" when looking to the left, seeing two images side-by-side. Convergence is generally preserved.

Action of superior oblique muscle

The anatomical basis of how to clinically test the superior oblique muscle and the trochlear nerve ... Trochlear nerve lesions are often missed in routine cranial nerve examinations. Unlike the oculomotor nerve, there is no effect on eyelids or pupils or detectable abnormal position at rest. The only symptom is often not being able to read clearly (blurred vision/diplopia on looking down and in). The eyeball will in fact be extorted, but this will not be detectable clinically. I will produce another video shortly which shows where the diplopia is maximal using the tradional 'H' pattern by controntation.
