The placement of implants in edentulous areas is often compromised by a thin alveolar crest. This reduction in the width of the edentulous ridge may make the final prosthetic results less than ideal. To correct this, various techniques have been described for widening, including revascularised block bone grafts, alveolar distraction osteogenesis, guided bone regeneration, and splitting to expand the ridge. However, these methods have limitations, including the need to harvest bone from intraoral or extr-oral sites along with increasing the time frame for the entire treatment during which the bone conditioning takes place which can be many months in a lot of cases before the implants can be placed. With the Osteotome Technique displayed in this video the implant osteotomy can be prepared without any ostectomy / bone removal and the implants can be inserted in the prepared sites . This is a Weil osteotomy of the 3rd metatarsal with screw fixation for a plantarflexed, painful metatarsal joint of the right foot. The patient had prior foot surgery on the 1st and 2nd metatarsals and had a relative plantarflexed 3rd metatarsal because of this. Conservative care did not ease pain, therefore surgery was elected. A 3.0 cannulated screw from Synthes was used for fixation.

Medical Korea The correction of severe sagittal deformity -World first osteotomy for spine deformity patient- Kyung Hee University International Medical Service A patient got a new life after several major operations of his spine. A 48- year- old patient visited Kyung Hee University Internal Medical service center with chin on chest deformity, severe thoracolumbar kyphosis and ankylosis of both hip joints. He was impossible to active daily living due to severe kyphosis, and his oral hygiene was very poor because of chin on chest deformity. Even looking straight ahead was an impossible dream for him over 15 years before the operations. The operations were conducted by Professor Kim Ki-tack, MD with other orthopedic surgery department staff in Kyung Hee University International Medical Service. The Kyphosis in his spine was over 165 degree and his spine deformity fixed after several operations over 7 months. He walked out of the hospital door with being able to be prone position, having a meal in sitting position, and looking straight ahead. All operations were conducted under bronchoscopic intubaton. As he was not able to be prone position, both hip resection arthroplasty was conducted first, and then cervical pedicle subtraction osteotomy was performed in sitting position 3 weeks later.
